Aftercare Planning

Recovery doesn't end at discharge. Our comprehensive aftercare planning ensures every client leaves with a structured, actionable plan for maintaining sobriety in the real world.

Planning for Life After Treatment

The transition from residential treatment back to everyday life is one of the most vulnerable periods in recovery. Without a solid aftercare plan, even motivated clients face significant risk of relapse when they return to familiar environments and stressors.

Aftercare planning at Connected Recovery Inc. begins at admission — not at discharge. From day one, our clinical team works with each client to identify the resources, support systems, and continuing care they will need to sustain their recovery long after they leave our facility.

Aftercare Components

  • Continuing care plan with scheduled follow-up appointments
  • Referrals to outpatient treatment programs (IOP/OP)
  • Sober living and transitional housing coordination
  • Connection to 12-Step and alternative recovery communities
  • Relapse prevention plan review and rehearsal
  • Medication management and psychiatric follow-up
  • Family reintegration planning and support
  • Employment, education, and life skills resources
