Planning for Life After Treatment
The transition from residential treatment back to everyday life is one of the most vulnerable periods in recovery. Without a solid aftercare plan, even motivated clients face significant risk of relapse when they return to familiar environments and stressors.
Aftercare planning at Connected Recovery Inc. begins at admission — not at discharge. From day one, our clinical team works with each client to identify the resources, support systems, and continuing care they will need to sustain their recovery long after they leave our facility.
Aftercare Components
- Continuing care plan with scheduled follow-up appointments
- Referrals to outpatient treatment programs (IOP/OP)
- Sober living and transitional housing coordination
- Connection to 12-Step and alternative recovery communities

Key Elements of a Strong Aftercare Plan
Continuing Treatment
Step-down levels of care including Intensive Outpatient (IOP) or Outpatient (OP) programs to maintain therapeutic momentum.
Recovery Support System
Connections to 12-Step meetings, SMART Recovery, Refuge Recovery, or other peer support communities matched to the client's preferences.
Safe Living Environment
Referrals to certified sober living homes or assistance with creating a substance-free home environment that supports recovery.
